WCCHC Health Providers can offer patients a holistic approach to health by providing referrals to our Food as Medicine programs. Patients can receive monthly vouchers to buy local fruits, vegetables and poi at our farmers market. Produce Prescription participants also receive health education and devices to monitor blood pressure and weight.

Keiki receive fresh local produce, healthy snacks and shelf-stable foods to take home.
KUPUNA PANTRYKupuna (62 years and older) receive fresh local produce, poi, healthy ready to eat meals, and pantry items.
Community Health Workers work with Kupuna to complete wellness assessments and provide resources, such as Medicaid and SNAP application assistance.

Our Nutrition & Lifestyle Services encompasses various programs and community events that focus on improving quality of life and preventing chronic conditions. This is accomplished through patient centered, culturally appropriate, and individualized services.
DIABETES SUPPORT
Patients can schedule one-on-one meetings with a
member of our health education team to develop
personalized strategies to self-manage diabetes.

WAIANAE OHANA PRODUCE PRESCRIPTION PROGRAM
Pregnant women and parents of children < 2 years old receive $60/month for 18 months.
KEIKI PRODUCE PRESCRIPTION PROGRAM
Children 2 - 8 years old receive $60/month for 6 months.
Children receive a monthly Food Activity Bag featuring a local produce of the month with recipes, nutrition lessons & activities.
FOOD SUBSCRIPTION PROGRAM
This program is for Asset Limited, Income Constrained, Employed (ALICE) households, who struggle to afford basic costs of living, yet do not qualify for financial assistance.
ALICE households receive $250/month for 6 months.
Vouchers can also use to buy local proteins & honey.
Food as Medicine program participants complete surveys throughout the programs to measure food insecurity, fruit and vegetable intake, and health-related quality of life. Data are tracked in participant's WCCHC Medical Records to connect good food with positive health outcomes.

EBT/ SNAP DOUBLE BUCKS PROGRAM
Mākeke provides unlimited doubling for EBT/ SNAP benefits towards the purchase of fresh local produce, poi, honey and proteins.
